To be trans works in a quite simple way, actually. When you are trans you want to be what you are, so your wish is, often but not always, to get a gender correcting operation. It can be the whole body, top and bottom or just top. Some people are non-op though.
This person who wants to have breasts so large they need to be carried around by breast male carriers is a funny figure. Could be part of a funny story. I don’t believe he exists in other words.

Sorry for the late reply but instead of doing magic how about talking to him about getting hormone replacement therapy. I suggest having him go through transition before doing anything magic related about it since in my opinion, it would only be a waste of energy.

This sounds less like a transgender person wanting to transition and more like weird fetishist fantasy porn (which is fine as long as it remains fiction). Pretty sure that if this person told of this wish to their gender therapist (mandatory in most places before hormones) they would either delay or deny hrt

Kindof how I saw this and no offense to his friend either.
There would be some in the trans community that would find this offensive no different than 50 shades of grey being offensive to BDSM.
If that’s his kink that’s cool, no judging but I wouldn’t call it a transition.
I agree that he should go through the therapy and hormones then after that consider magick. I feel it would work better for him. There are a few Deities out there that can help with a true transition when he wants it, all he has to do is call them.

Off my head I’d recommend Loki.
He is known for his fluidity and exceptance.
He is said to take it that which is considered broken and can truly love and understand them.
He also has no problem kicking them in a direction that he feels will best help them.
